Chronic Disease Program (CDSMP)

The Chronic Disease Program (CDSMP), an evidence-based program developed at Stanford University Patient Education Research Center as a collaborative with the Northern California Kaiser Permanente Medical Care Program, is proven to help participants with chronic co-existing conditions learn decision-making, better breathing, dealing with pain, dealing with difficult emotions, and more. This program is made available for people 60 years of age and older with diabetes, hypertension, heart disease, stroke, arthritis, or any other chronic conditions and their caregivers. Meet with your Certified CDSMP Leader for 1 hour once weekly for 6 weeks.
