Chronic Disease Self-Management Program (CDSMP)

An evidence-based program developed at Stanford University Patient Education Research Center as a collaborative with the Northern California Kaiser Permanente Medical Care Program, this program is proven to help participants with chronic co-existing conditions learn decision-making, better breathing, dealing with pain, dealing with difficult emotions, and more.
This program is made available for people 65 years of age and older with diabetes, hypertension, heart disease, stroke, or arthritis.
